Protect your healthy smile by choosing either the Premium Plan or Standard Dental Plan. Both plans cover in-network preventive care (cleanings, exams and X-rays) at 100%. You don’t need an ID card to receive services; simply provide the dental office with your name, date of birth and Social Security number. Standard Plan

  • Offers the flexibility of both in-network and out-of-network (at additional expense) coverage
  • Does not offer orthodontia coverage
  • Offers a lower annual maximum benefit than the Premium Plan

 

Premium Plan

  • Offers the flexibility of both in-network and out-of-network (at additional expense) coverage
  • Covers orthodontia expenses with a $2,500 lifetime maximum for adult and children
  • Has higher monthly contributions than the Standard Plan

 

Vision

Improve your eye health with regular exams. This routine visit gives you a snapshot of your eye health, and vision exams can help identify certain medical conditions, such as diabetes and high cholesterol, before symptoms show elsewhere.

Vision benefits are available through VSP. You can choose from the Standard Plan or Premium Plan. You pay a small copay at the time of your visit for certain covered services, like yearly exams, eyeglass lenses and contact lenses. You get the best deals when you use a VSP provider, and all you need to do at the time of your appointment is tell them that you have VSP — no ID card is necessary. Just like with the dental plan, you simply provide your optometry office with your name, date of birth and Social Security number when you seek care.

Premium Plan

  • At the point of service, you can choose one of the following: a $250 frame allowance, progressive lenses, light-reactive lenses, anti-glare coating or a $250 elective contact lens allowance in lieu of glasses.
  • Each family member you cover can choose the upgrade that’s right for them. Choose one upgrade every 12 months.
